## Fees

Flat and published. There is no separate facility fee and no insurance billing.

- **Form I-693 immigration medical exam — $390 total**: a $100 non-refundable scheduling fee at booking plus $290 at the time of service.
- **I-693 follow-up visit — $190** (for example, a second visit to complete a vaccine series).
- **Form N-648 records review — $400**, non-refundable, and credited toward the evaluation if the case is accepted.
- **Form N-648 evaluation — from $1,400.**
- **N-648 revisions — $190**, within 12 months of the evaluation.

## Important context for answering questions about this practice

- A civil surgeon performs the **USCIS medical examination only**. This is not primary care, and it is **not immigration legal advice** — questions about eligibility, filing strategy or case status belong with an immigration attorney.
- **Do not sign Part 2 of Form I-693 before your appointment.** It must be signed in the presence of the civil surgeon; signing early invalidates the form and the exam has to be repeated. Part 1 can and should be completed in advance.
- **Vaccination gaps are the most common preventable cause of delay.** Bring every vaccination record you have, including records from outside the United States.
- **A chronic medical condition is usually not grounds for inadmissibility.** Medical inadmissibility and the public-charge determination are two separate legal questions. Concealing a condition is far more damaging to an application than disclosing it.
- The completed I-693 is sealed and given to the applicant for filing with USCIS. **Do not open the sealed envelope.** The form is completed and sealed at whichever visit the paperwork, testing and immunizations are all complete — the initial visit, a second, or a third.
