INTERVENTIONAL CENTER FOR PAIN MANAGEMENT, P.C. DBA ImmigrationExam.us is operated by a licensed medical clinic providing USCIS I-693 immigration medical examinations and related clinical services in the State of Missouri. By accessing this website, scheduling an appointment, submitting forms, or communicating with our clinic, you agree to the following terms.
Medical Services Disclaimer
All medical services are provided only after an in-person evaluation by a licensed healthcare professional. Information on this website is for general informational purposes only and does not constitute medical advice. Use of this website does not establish a physician-patient relationship outside of an in-clinic encounter.
Appointments, Fees, and Payments
Service fees are disclosed before scheduling. Payment may be required in advance to reserve an appointment. All fees charged are nonrefundable, including fees for missed appointments, cancellations, or failure to complete required documentation.
We offer a 100% Money Back Guarantee if your Form I-693 is rejected solely due to our error in paperwork, and you have fully complied with all vaccination and medical requirements—and there are no other defects in your application—you’re entitled to a full refund. You must submit a complete copy of the official USCIS letter or Notice stating the rejection. Confirm there are no other application errors—such as missing documents, legal inconsistencies, or previously submitted components. If any other issues exist, we cannot issue a refund.
Scope of Money Back Guarantee. Our 100% Money Back Guarantee applies exclusively to USCIS Form I-693 immigration medical examination services performed at our office. It does not apply to Form N-648 disability waiver medical evaluations, records review and case-acceptance fees, revision fees, interpreter fees, or any other service. N-648 evaluation fees compensate the physician for professional time, records analysis, cognitive assessment, and documentation — not for any particular result. USCIS alone determines whether a Form N-648 is sufficient, and no refund is owed based on USCIS’s decision. The $400 records review / case-acceptance fee is non-refundable in all circumstances; if your case is accepted, it is credited in full toward your evaluation fee.
By scheduling, you acknowledge and agree that immigration medical examinations involve reserved clinical time, administrative preparation, and regulatory documentation.
No Guarantee of Immigration Outcome
Completion of a medical examination does not guarantee USCIS approval or any immigration benefit. The clinic’s role is limited to performing required medical evaluations and completing USCIS-mandated documentation.
Electronic Communications and SMS Consent
By providing your phone number or email address, you consent to receive electronic communications from ImmigrationExam.us related solely to appointment scheduling, confirmations, reminders, required documentation, exam follow-up, and care coordination. Message frequency varies. Message and data rates may apply. You may opt out of SMS communications at any time by replying STOP.
SMS Messaging Terms of Service (A2P 10DLC Compliance)
By providing your mobile phone number and opting in to receive SMS (text) messages, you agree to the following terms regarding SMS communications from INTERVENTIONAL CENTER FOR PAIN MANAGEMENT, P.C. DBA ImmigrationExam.us.

Opt-Out Instructions: You may revoke your consent and stop receiving SMS messages at any time by replying STOP to any SMS message received from us. After processing your STOP request, we will send a one-time confirmation, and no further SMS messages will be sent, except for required administrative or legal notifications. For assistance, you may reply HELP or contact customer support as described below.

We will honor requests promptly. We share your mobile number only with our SMS delivery provider for the sole purpose of delivering messages on our behalf. That provider is contractually prohibited from using this information for its own marketing or other unrelated purposes.

Limitation of Liability
To the fullest extent permitted by Missouri law, INTERVENTIONAL CENTER FOR PAIN MANAGEMENT, P.C. DBA ImmigrationExam.us shall not be liable for indirect, incidental, or consequential damages arising from use of this website or scheduling services.
