The rest belongs to the civil surgeon, who completes it at your immigration medical exam. Leave Part 2 unsigned. You sign only when the civil surgeon instructs you during the exam.
When do I sign Form I-693?
Only during your exam, when the civil surgeon instructs you to. Complete your information in Part 1 ahead of time, but do not sign Part 2 in advance: a premature signature can invalidate the form. Submit Part 1 on this page, unsigned, and the civil surgeon will tell you when to sign as part of your immigration medical exam.
