Immigration Medical Exam (I-693) in St. Louis City
City residents adjusting status complete the Form I-693 immigration medical exam with a DHS-designated civil surgeon. We are a short, familiar drive from downtown. The form is sealed the same day when everything is in order.
A city drive, not a trip
St. Louis is an independent city. Its residents use a different set of public health services than the county around it. That difference matters mostly when you need a vaccine record. The exam itself is simple. It is fifteen to twenty minutes from most city neighborhoods to the Woodson clinic by the airport.
What is done at the visit
The visit covers your history and the exam USCIS defines. It includes an immunization review (a check of your vaccines), and any missing doses are given. Tuberculosis, syphilis and gonorrhea tests are done on site. Budget 2 to 6 hours. Costs and turnaround are posted on fees and timeline. The dose rules by age are on vaccine requirements.
The designation is the whole point
An I-693 signed by a physician who is not designated comes back as a Request for Evidence months later. Dr. Gurpreet Singh Padda, MD, MBA, MHP is a DHS-designated civil surgeon, CSID 111051. He is board-certified by five American specialty boards. The designation is federal.
Getting here from the city
4477 Woodson Rd, Suite 102 is 15 to 20 minutes via I-64 or I-70 from most city neighborhoods. It sits right beside Lambert Airport, with easy parking.
County residents
Your address may be in St. Louis County, not inside the city limits. If so, the county health centers are your public vaccine option. Those are listed on the St. Louis County page.
When the civics test is the barrier
Some applicants have a medical or mental-health condition that keeps them from completing the English and civics requirement. That condition may support an N-648 disability waiver. It is a separate evaluation, done in house. We do it in the applicant’s own language where that gives a more accurate assessment.
City immunization records and exemptions
Missing vaccines, or ones you cannot prove, delay more I-693s than anything else. Doses can be given at your exam. But you may need a copy of a record the city already holds, or an exemption. For those, the City of St. Louis runs its own services, separate from the county.

Frequently asked questions
How do I get a copy of a vaccination record the city holds?
The City of St. Louis records line is (314) 657-1548. It is open 8:30 a.m. to 5:00 p.m. weekdays. The fee is $10 per record, payable in cash. Bring whatever they release to your exam.
A record issued outside the United States is a different question. It is answered in Will USCIS Accept My Foreign Vaccination Records? What Counts for the I-693.
How long does a completed I-693 stay valid?
The rules on validity, and on when the form must be signed, have shifted more than once lately. That is why we date and seal at completion, not in advance. The current position is summed up in what changed on the I-693 since 2025.
Do I need the gonorrhea and syphilis testing?
It depends on age and on the current USCIS requirements. That is why it is drawn on site, not assumed. Who gets tested and why: gonorrhea and syphilis testing in the I-693 process.
Will I be charged for a second visit?
A follow-up is $190, and only when one is truly needed. That means for a medical reason, or because the first visit was missing paperwork, testing or immunization records. A complete first visit carries no follow-up fee. See fees and timeline.
